1.  Requires invoice from original sale to support payout amount.

2.  Will require a Bill of Sale from you to GreatAmerica.

3.  Lien searches on existing equipment when payment exceeds $25,000.

4.  Subordinations from prior filers will be required if UCC's are filed as a blanket lien on all company assets or filed specifically on the equipment we will be re-financing (in this case a UCC Termination would be required).

5.  Need to ensure existing phone system being transitioned to the new lease is listed in addition to the new equipment.
